Abstract

A percutaneous urinary catheter is provided, including a proximal portion, a distal portion configured to be deployed in a kidney and/or renal pelvis of the urinary tract, and at least one lumen extending at least partially through the proximal portion of the percutaneous urinary catheter; and a negative pressure source in fluid communication with the at least one lumen, the negative pressure source being configured to draw fluid from the urinary tract into the at least one lumen of the percutaneous urinary catheter for removal of the fluid from the urinary tract.

Claims (55)

1. A system for fluid removal from a portion(s) of a urinary tract, the system comprising:

a percutaneous urinary catheter comprising a proximal portion, a distal portion comprising a retention portion configured to be deployed in a kidney and/or renal pelvis of the urinary tract, and at least one lumen extending at least partially through the proximal portion of the percutaneous urinary catheter; and

a negative pressure source in fluid communication with the at least one lumen, the negative pressure source being configured to draw fluid from the urinary tract into the at least one lumen of the percutaneous urinary catheter for removal of the fluid from the urinary tract,

wherein the percutaneous urinary catheter is configured to inhibit mucosal tissue from appreciably occluding one or more drainage hole(s), port(s), opening(s), or perforation(s) in the retention portion when negative pressure from the negative pressure source is applied through the percutaneous urinary catheter.

2. The system of claim 1 , wherein the proximal portion of the urinary catheter is configured to pass through a percutaneous opening.

The system of claim 1 , wherein the one or more drainage hole(s), port(s), opening(s), or perforation(s) are disposed on a protected surface area or inner surface area of the retention portion of the percutaneous urinary catheter, and wherein an outer periphery or protective surface area of the retention portion of the percutaneous urinary catheter is configured to support mucosal tissue and thereby prevent occlusion of the one or more drainage hole(s), port(s), opening(s), or perforation(s) upon application of negative pressure through the percutaneous ureteral catheter.

4. The system of claim 3 , wherein the outer periphery or protective surface area of the retention portion of the percutaneous urinary catheter is essentially free or free of the one or more drainage hole(s), port(s), opening(s), or perforation(s).

5. The system of claim 1 , wherein the retention portion is configured to be deployed to retain the distal portion of the percutaneous urinary catheter in the kidney and/or renal pelvis of the urinary tract.

6. The system of claim 5 , wherein the retention portion comprises a proximal end sized to be positioned in the kidney and a distal end sized to be positioned in a uretero-renal pelvis junction.

7. The system of claim 5 , wherein the retention portion comprises a coiled retention portion comprising at least a first coil having a first diameter and at least a second coil having a second diameter, the first diameter being greater than the second diameter.

8. The system of claim 7 , wherein the first coil is closer to the proximal portion of the catheter than is the second coil.

9. The system of claim 5 , wherein the retention portion is configured to be extended into a deployed position in which a diameter of the retention portion is greater than a diameter of the drainage lumen portion.

The system of claim 1 , wherein the retention portion comprises an outer periphery or protective surface area that inhibits mucosal tissue from the kidney, the renal pelvis, and/or a uretero-renal pelvis junction from occluding the one or more drainage hole(s), port(s), opening(s), or perforation(s) of the retention portion upon application of negative pressure through the catheter.

The system of claim 1 , wherein the retention portion comprises one or more helical coils, each coil having an outwardly facing side and an inwardly facing side, and wherein the one or more drainage hole(s), port(s), opening(s), or perforation(s) that permits fluid flow into the at least one lumen of the proximal portion are disposed on the inwardly facing side(s) of the one or more helical coil(s).

The system of claim 1 , wherein a number of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a distal end of the retention portion is greater than a number of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a proximal end of the retention portion.

The system of claim 1 , wherein a size of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a distal end of the retention portion is greater that a size of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a proximal end of the retention portion.

The system of claim 1 , wherein a total area of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a distal end of the retention portion is greater than a total area of the one or more drainage hole(s), port(s), opening(s), or perforation(s) towards a proximal end of the retention portion.

The system of claim 1 , wherein the retention portion is configured such that, when the negative pressure source provides negative pressure through the at least one lumen of the percutaneous urinary catheter, fluid is drawn into the one or more drainage hole(s), port(s), opening(s), or perforation(s) while mucosal tissue is prevented from appreciably occluding the one or more drainage hole(s), port(s), opening(s), or perforation(s).

16. The system of claim 1 , wherein the negative pressure source is configured to induce a negative pressure in a portion(s) of the urinary tract, which causes the fluid from the urinary tract to be drawn into the at least one lumen of the percutaneous urinary catheter.

17. The system of claim 1 , wherein the negative pressure source provides negative pressure at a proximal end of the proximal portion of the percutaneous urinary catheter within a range of about 0.1 mmHg to about 150 mmHg.

18. The system of claim 1 , further comprising a controller configured to cause the negative pressure source to provide negative pressure or positive pressure.

19. The system of claim 1 , wherein the negative pressure source is a pump.

20. The system of claim 19 , wherein the pump has an accuracy of about 10 mmHg or less.

21. The system of claim 19 , wherein the pump comprises at least one of a diaphragm aspirator pump, a peristaltic pump, or a piston pump.

22. The system of claim 19 , wherein the pump is configured to alternate between generating negative pressure and generating positive pressure.

23. The system of claim 22 , wherein the pump provides positive pressure at the proximal end of the percutaneous urinary catheter within a range of from about 0.1 mmHg to about 150 mm Hg.

24. The system of claim 19 , further comprising a controller configured to regulate positive and/or negative pressure provided by the pump within a pressure range that facilitates increased urine production from the kidney.

25. The system of claim 24 , wherein the controller is configured to cause the pump to operate intermittently to emit pulses of the positive and/or negative pressure to the at least one lumen of the percutaneous urinary catheter.

The system of claim 24 , further comprising at least one sensor electrically connected to the controller, wherein the sensor is configured to sense signals representative of a physical condition of a patient in which the percutaneous urinary catheter is deployed or a fluid characteristic of fluid passing through the at least one lumen of the percutaneous urinary catheter.

27. The system of claim 26 , wherein the fluid characteristic sensed by the at least one sensor comprises at least one of conductance, flow volume, temperature or analyte concentration of the fluid drawn into the at least one lumen of the percutaneous urinary catheter.

28. The system of claim 26 , wherein the at least one sensor is embedded in a sidewall of the percutaneous urinary catheter or positioned in a fluid collection container fluidly connected to the at least one lumen of the percutaneous urinary catheter.

29. The system of claim 26 , wherein the controller is configured to receive and process the signals from the at least one sensor and adjust at least one operating parameter of the pump based, at least in part, on the received and processed signals.

30. The system of claim 29 , wherein the controller is configured to adjust the at least one operating parameter of the pump by:

determining a value for at least one physiological parameter of the patient based, at least in part, on the received and processed signals;

comparing the determined at least one physiological parameter value to a clinically acceptable range of values for the at least one physiological parameter; and

actuating the pump when the determined at least one physiological parameter value is outside of the clinically acceptable range of values.

The system of claim 30 , wherein the at least one physiological parameter comprises at least one of hematocrit concentration of blood, protein concentration of blood or urine, creatinine concentration of blood or urine, blood pressure, blood flow velocity, blood oxygen saturation, heart rate, respiration rate, or respiratory gas concentration of the patient.

The system of claim 26 , further comprising a wireless transmitter electronically coupled to the controller, wherein the controller is configured to cause the wireless transmitter to transmit information about a condition of the patient sensed by the at least one sensor and/or treatment provided to the patient by the pump to a remote system or device.

The system of claim 19 , wherein the pump is configured to be positioned external to a body of a patient in which the percutaneous urinary catheter is deployed, and wherein the proximal portion of the percutaneous urinary catheter is connected to the pump and extends from the pump through a percutaneous opening to the urinary tract of the patient.

34. The system of claim 1 , wherein the retention portion comprises a coiled retention portion comprising a plurality of coils, and wherein a distal-most coil of the plurality of coils has a smaller diameter than other coils of the plurality of coils.

35. A method for removing fluid from a portion(s) of a urinary tract, the method comprising:

inserting a distal portion of a percutaneous urinary catheter through a percutaneous opening into a kidney and/or renal pelvis of the urinary tract;

deploying the distal portion of the percutaneous urinary catheter within the kidney and/or renal pelvis for at least a duration of a negative pressure therapy treatment;

connecting a proximal end of the percutaneous urinary catheter to a negative pressure source; and

causing the negative pressure source to exert negative and/or positive pressure to at least one lumen of the percutaneous urinary catheter to draw fluid from the urinary tract into the at least one lumen of the percutaneous urinary catheter, wherein the percutaneous urinary catheter is configured to inhibit mucosal tissue from appreciably occluding one or more drainage hole(s), port(s), opening(s), or perforation(s) in the retention portion when negative pressure from the negative pressure source is applied through the percutaneous urinary catheter.

36. The method of claim 35 , wherein deploying the distal portion of the percutaneous urinary catheter within the kidney and/or renal pelvis retains the distal portion of the percutaneous urinary catheter within the kidney and/or the renal pelvis.

37. The method of claim 35 , wherein the negative pressure source is an external pump positioned outside of the percutaneous opening.

38. The method of claim 37 , wherein causing the negative pressure source to exert the negative and/or positive pressure comprises activating the pump.

39. A catheter configured to be deployed in a urinary tract of a patient, comprising:

a proximal portion configured to pass through a single percutaneous opening only; and

a distal portion comprising a retention portion configured to be deployed in a kidney and/or renal pelvis of the patient,

wherein the retention portion is configured to inhibit mucosal tissue from appreciably occluding one or more drainage holes in the retention portion of the distal portion of the percutaneous catheter when negative pressure is applied through the catheter.

40. A system for fluid removal from a portion(s) of a urinary tract, the system comprising:

a catheter according to claim 39 ; and

a negative pressure source in fluid communication with at least one lumen of the proximal portion, the negative pressure source being configured to draw fluid from the urinary tract into the at least one lumen of catheter for removal of the fluid from the urinary tract.
