Abstract

A system and methodology for the preservation of red blood cells is described in which red blood cells are oxygen or oxygen and carbon dioxide depleted, treated and are stored in an anaerobic environment to optimize preparation for transfusion. More particularly, a system and method for extended storage of red blood cells from collection to transfusion that optimizes red blood cells prior to transfusion is described.

Claims (43)

1 . A method for editing and storing red blood cells comprising:

obtaining whole blood;

separating red blood cells from the whole blood to form packed red blood cells;

depleting oxygen from the packed red blood cells to form depleted packed red blood cells, wherein the depleting comprises passing the packed red blood cells through a device having a disposable housing, an inlet port, an outlet port, one or more gas permeable membranes adapted to receiving and conveying red blood cells, and an oxygen sorbent;

leukoreducing the whole blood, the packed red blood cells, or the depleted packed red blood cells;

storing the depleted packed red blood cells under anaerobic conditions in an anaerobic storage bag comprising an outer bag having a barrier film that is impermeable to oxygen and an inner bag in contact with the red packed blood cells to form stored depleted red blood cells; and

editing the stored depleted packed red blood cells to form stored edited and depleted packed red blood cells, wherein the editing comprises identifying and removing moribund, dead, or dying red blood cells from the stored depleted packed red blood cells.

2 . The method of claim 1 , wherein the editing comprises removing the densest red blood cells from the packed red blood cells by centrifugation or filtration.

3 . The method of claim 1 , wherein the editing comprises exposing the stored depleted packed red blood cells to a biomechanical shock, and eliminating red blood cells damaged by the biomechanical shock.

4 . The method of claim 3 , wherein the biomechanical shock is osmotic shock.

5 . The method of claim 1 , wherein the editing comprises filtering the stored depleted packed red blood cells with a filter having a high affinity ligand capable of binding red blood cells having a desired surface marker.

6 . The method of claim 5 , wherein the desired surface marker is phosphatidylserine or aggregated protein 3.

7 . The method of claim 5 , wherein the high affinity ligand is Annexin IV or an antibody against the desired surface marker.

8 . The method of claim 5 , wherein the high affinity ligand is conjugated to a multimeric molecule to form aggregates of red blood cells having the desired surface marker, and whereby the aggregates are removed by filtration or centrifugation.

9 . The method of claim 1 , wherein the editing comprises separating red blood cells with reduced deformability from the stored depleted packed red blood cells by processing through a bump array microfluidic device.

10 . The method of claim 1 , wherein the editing is with an editing device, wherein the editing device is selected from the group consisting of:

a filter to remove the densest red blood cells;

a filter having a high affinity ligand capable of binding red blood cells having a desired surface marker;

a bump array device for separating red blood cells with reduced deformability; and

any combination thereof.

11 . The method of claim 1 , further comprising depleting carbon dioxide from the packed red blood cells, and wherein the barrier film of the outer bag is impermeable to both oxygen and carbon dioxide.

12 . The method of claim 1 , wherein an oxygen sorbent or an oxygen and carbon dioxide sorbent is disposed between the outer and inner bags.

13 . The method of claim 1 , wherein transfusion of the stored edited and depleted packed red blood cells into a patient in need thereof reduces excess iron burden in the patient.

14 . The method of claim 13 , wherein the patient is a chronically transfused patient or a massively transfused patient.

15 . The method of claim 1 , wherein transfusion of the stored edited and depleted packed red blood cells into a patient in need thereof reduces the risk of a capillary blockage in the patient.

16 . The method of claim 1 , further comprising one or more steps selected from the group consisting of:

removing platelets from the whole blood or the packed red blood cells;

adding an additive solution to the packed red blood cells;

irradiating the whole blood, the packed red blood cells, the depleted packed red blood cells, the stored and depleted packed red blood cells, or the stored edited and depleted packed red blood cells;

inactivating pathogens in the whole blood, the packed red blood cells, the depleted packed red blood cells, the stored and depleted packed red blood cells, or the stored edited and depleted packed red blood cells;

adding nitric oxide or a nitric oxide precursor molecule to the stored edited and depleted packed red blood cells;

reducing the volume of the stored edited and depleted packed red blood cells after storage in the anaerobic storage bag; and

reoxygenating the stored edited and depleted packed red blood cells for transfusion into a patient in need thereof.

17 . The method of claim 1 , wherein the storing is for a period of about 7 weeks to about 15 weeks.

18 . The method of claim 17 , wherein the storing is for a period of about 10 weeks to about 15 weeks.

19 . A method for editing and storing red blood cells comprising:

obtaining whole blood;

separating red blood cells from the whole blood to form packed red blood cells;

editing the packed red blood cells to form edited packed red blood cells, wherein the editing comprises identifying and removing moribund, dead, or dying red blood cells from the packed red blood cells;

depleting oxygen from the edited packed red blood cells to form edited and depleted packed red blood cells, wherein the depleting comprises passing the packed red blood cells through a device having a disposable housing, an inlet port, an outlet port, one or more gas permeable membranes adapted to receiving and conveying red blood cells, and an oxygen sorbent;

leukoreducing the whole blood, the packed red blood cells, the edited packed red blood cells, or the edited and depleted packed red blood cells; and

storing the edited and depleted packed red blood cells under anaerobic conditions in an anaerobic storage bag comprising an outer bag having a barrier film that is impermeable to oxygen and an inner bag in contact with the red packed blood cells to form stored edited and depleted packed red blood cells.

20 . The method of claim 19 , further comprising depleting carbon dioxide from the edited packed red blood cells, wherein the barrier film of the outer bag is impermeable to both oxygen and carbon dioxide.
