Abstract

Disclosed herein are systems, devices, and methods for the removal of objects from the body. The device may be a urethral catheter configured to aspirate kidney stones from the urinary tract through one or more aspiration ports at the distal face or along a lateral side of the catheter. The catheter may include one or more irrigation ports at the distal face or along the lateral side of the catheter for dislodging kidney stones. The device may be steerable. The spatial arrangement of the one or more irrigation ports with respect to the one or more aspiration ports may vary. The device may include an irrigation tube and/or a shield member configured to spatially confine the kidney stones adjacent the catheter. Various temporal patterns of aspiration and irrigation are disclosed for optimizing removal of kidney stones.

Claims (35)

1 . A procedure of removing a kidney stone or fragments of the kidney stone from a kidney of a patient, the method comprising:

irrigating the kidney by applying an irrigation fluid through an irrigation lumen;

removing the kidney stone or fragments of the kidney stone from the kidney through a vacuum lumen by applying suction through the vacuum lumen,

wherein

the irrigation fluid is applied at flow rates comprising a first flow rate (ml/min) in a first flow rate range (ml/min) and a second flow rate (ml/min), different than the first flow rate, in a second flow rate range (ml/min), different than the first flow rate range,

the first flow rate in the first flow rate range is used for either one or two of the following modes (i)-(iii) of the procedure

(i) while inserting a distal segment of the irrigation lumen into the kidney,

(ii) while removing the distal segment from the kidney,

(iii) while the distal segment resides within the kidney, and

the second flow rate in the second flow rate range is used for either the one remaining mode (i)-(iii) of the procedure or the two remaining modes (i)-(iii) of the procedure for which the first flow rate in the first flow rate range is not used.

2 . The procedure of claim 1 , wherein for the mode (iii) of the procedure, the distal segment is positioned within a calyx of the patient.

3 . The procedure of claim 1 , wherein the first flow rate fluctuates within the first flow rate range and the second flow rate fluctuates within the second flow rate range.

4 . The procedure of claim 1 , wherein irrigation fluid is applied continuously through the irrigation lumen until irrigation fluid returns through the vacuum lumen to outside of the patient.

5 . The procedure of claim 1 , wherein for the mode (iii) of the procedure the distal segment is positioned within a first calyx of the kidney and the method optionally comprises:

positioning the distal segment within another calyx of the kidney;

optionally removing another kidney stone or fragments of the another kidney stone from the another calyx through the vacuum lumen by applying suction through the vacuum lumen,

wherein

the irrigation fluid is applied at a third flow rate (ml/min) while the distal segment is being inserted, while the distal segment is being removed, or while the distal segment is within the another calyx, wherein the third flow rate is within the range of the first or second flow rate ranges but is different than the first and second flow rates.

6 . The procedure of claim 5 , further comprising removing the another kidney stone or fragments of the another kidney stone from the another calyx by applying suction through the vacuum lumen.

7 . The procedure of claim 1 , additionally comprising breaking apart the kidney stone into the fragments with the use of a laser.

8 . A procedure of removing a kidney stone or fragments of the kidney stone from a kidney of a patient, the method comprising:

irrigating the kidney by applying an irrigation fluid through an irrigation lumen;

removing the kidney stone or fragments of the kidney stone from the kidney through a vacuum lumen by applying suction through the vacuum lumen,

wherein

the irrigation fluid is applied at flow rates comprising a first flow rate (ml/min) in a first flow rate range (ml/min) and a second flow rate (ml/min), different than the first flow rate, in a second flow rate range (ml/min), different than the first flow rate range,

the first flow rate in the first flow rate range is used for any of the following modes (i)-(iii) of the procedure

(i) while inserting a distal segment of the irrigation lumen into the kidney

(ii) while removing the distal segment from the kidney,

(iii) while the distal segment resides within the kidney, and

the second flow rate in the second flow rate range is used to remove the kidney stone or fragments of the kidney stone.

9 . The procedure of claim 8 , wherein removing the kidney stone or fragments of the kidney stone includes fluidizing the kidney stone or fragments of the kidney stone with the irrigation fluid in the second flow rate range.

10 . The procedure of claim 8 , wherein removing the kidney stone or fragments of the kidney stone includes dislodging the kidney stone or fragments of the kidney stone with the irrigation fluid in the second flow rate range.

11 . The procedure of claim 8 , wherein the first flow rate fluctuates within the first flow rate range and the second flow rate fluctuates within the second flow rate range.

12 . The procedure of claim 8 , wherein the irrigation lumen comprises one or more lumens in communication with one or more irrigation ports, the one or more irrigation ports are configured to radially project the irrigation fluid.

13 . The procedure of claim 8 , additionally comprising breaking apart the kidney stone into the fragments with the use of a laser.
